The Pac-Man kill screen is caused by the level counter overflowing to 0. The level counter wasn‘t designed to handle 0, and thus keeps drawing level icons until that counter hits 1, going around the overflow itself. Well after that point, it has ran out level icons and is drawing grabage, and has drawn over half the play field. The level can‘t be beaten, as there aren‘t enough dots.

It was once possible to headshot someone through thier foot. TF2 used to check which body part you hit, but not if that that body part was attached to thing you hit. TF2 alreadly knows who you hit, as the shot has impacted the cheaper-to calculate hit-box around them. Hence, a shot through a player‘s bounding box that lands on a teleporter‘s body-part 1, was a shot to the player body-part 1: Thier head.
